Slight addition: the CD-ROM version works fine too. Google Translate's camera function really helped out there. (Though you have to load the Japanese language in DOSBox first or the text is gibberish.) Basic form is press 2 for IBM mode instead of PC-88 at the first screen, enter on the message about creating a floppy disk, enter on the bottom-most option (it will install to C:\PUYO2 by default, which is fine) and for some reason it defaults to cancel so left and enter to select continue. You'll probably want to set it to use SB-16 audio. (It seems there is no MIDI configuration so you're stuck with FM synthesis sadly. I don't know why they even used a CD-ROM with audio tracks and then didn't even make CDDA tracks for the game's music.)

I tried the rip version first and it seemed to run fine. Maybe you don't have enough cycles set? I am using DOSBox-X, but I don't think it would make a difference for this game. Modern computers are really fast now, so I go ahead and emulate a pentium 1 level machine with pentium_mmx type and fixed 52000 cycles and the only thing that doesn't run fine seems to have some sort of bugs or something rather than the system not keeping up. This is fast enough for pretty much any game that says it needs a pentium for maximum quality settings. If in doubt though, try setting cycles=max instead.

Puyo Puyo 2 is a great sequel to Puyo Puyo, one of the best Tetris-style game that inspired numerous clones on its own.
The gameplay remains the same as the first game: colored cartoon jelly blobs fall from the sky in pairs, and your goal is to connect four blobs of the same color together to make them explode, making any blob above them to fall down. At the same time, you must compete against an opponent (either computer or another human player) who is trying to do the same thing. Every time a group of jellies explode, a transparent jelly will fall in your opponent's playing field, making his/her job more difficult. You can also set up chain reactions by arranging the blobs in such a way that one color combo will lead to multiple combos as blobs fall into place. Puyo Puyo 2 adds new gameplay options and special blobs. One of the most devious additions is the double transparent blob, which require TWO color combos to vaporize. Compile has also added an option to watch matches between computer experts, and more multiplayer options.
Overall, if you like Puyo Puyo, you will love this sequel. And if you have never tried the game before, you can’t miss this fun underdog that was never released in Europe and the U.S. Two thumbs up, way up!
Note: the version for download here is the Korean version, published by Korea division of Compile. Unfortunately there is no English version of any Puyo Puyo games (now up to the fourth, released for the Dreamcast).
